To obtain a PUP-CHC proposal, please fill out the Proposal Request Form (72KB PDF) or provide us with the requested information. You can mail the request, fax it to (616) 454-4338 or e-mail it back to PUP-CHC.

Before requesting a proposal, please note the following requirements for groups in the PUP-CHC plan:

Eligibility Requirements:

  • Must be a member of the Central, Southeastern or Western Chapter of the Associated Builders and Contractors of Michigan, Inc.
  • Groups of 2 to 5 employees – 100% of all eligible employees must participate.
  • All groups over 6 employee lives require 75% of all full-time eligible employees.
  • 50% of all full-time employees and their dependents must participate for any size group.
  • Full-time employment requires a minimum of 27.5 hours per week.

Co-existing
  • PUP-CHC will co-exist with an HMO only.
  • 75% participation of all eligible employees and dependents is required.


Contribution

PUP-CHC requires the employers to pay a minimum of 25% of the premium cost.
