PPO Medical Plans:

Deductible choices for these plans are $250, $500, $1000, $2000, $3000, $4000 and $5000 In-Network and 3X Non-Network.

These plans also offer a choice of coinsurance (% of $12,500 or % of $15,000 or % of $20,000) and co-pays ($20 or $30).  The deductibles, OOP limit and maximums can be combined or separate for both the network and non-network.

HSA Plans

  • HSA 100 (deductibles and coinsurance combined in and out of network)
  • HSA 100-2 (deductibles and coinsurance separate in and out of network)
  • HSA 80 (deductibles and coinsurance combined in and out of network)
  • HSA 80-2 (deductibles and coinsurance separate in and out of network)

Networks

  • All medical plans incorporate a PPO format. Greater coverage for in-network vs. out-of-network.
  • Cofinity Network in Michigan.
  • MultiPlan/PHCS as our National Network.
  • Envision Rx, a National PBM.

Prescription Drug Plans
Prescription Drug Plan Options (56KB PDF)

  • All PUP of Michigan medical plans require prescription drug coverage.
  • All PPIs and non-sedating antihistamines with over-the-counter (OTC) equivalents will not be covered.
  • Covered injectables will require preauthorization and are subject to a twenty-five percent (25%) coinsurance. The maximum participant contribution will not exceed $3,500 per calendar year.
  • National Coverage of over 60,000 pharmacies.
  • All mail orders will be two times the retail co-pay program chosen.

Life Insurance

  • A minimum of $15,000 of life insurance is required for every covered employee taking the medical program.
  • Dependent life is an option (see schedule).

COBRA

  • Complete COBRA administration for groups with twenty (20) or more employees.
  • Setting of COBRA rates, notification letters, coupon booklets, six month reminders of COBRA ending and most important, collection of premium. All is done by Preferred United Plans.
  • Fees are $0.75 per employee per month. Upon election of COBRA, Preferred United Plans of Michigan retains a 2% premium surcharge.
  • This is an option to the employer, and must be selected on the employer application.
