Required Information for a Presentation/Proposal
SecureOne will work with the broker and agent community, if requested. We do, however, require that we be present when our proposal is presented to the prospective client.

To properly obtain a completed proposal we will require the following information:

  • Copy of current plan document or policy
  • Complete listing of all employees and dependents covered under the plan or plans
  • If there are divisions, a list of divisions, and which employees are in which division
  • Current networks used by each division, if required
  • Two years of claims history by month and by line of coverage (medical, dental, rx, etc.)
  • Two years of individual claims that have exceeded the 50% threshold of the specific stop-loss level.
  • Attending physician or case management notes for each on of the individuals who have reached the 50% stop-loss level.
  • Two years of rate history.
  • Copy of the current stop-loss contract with rates, contract language, specific and aggregate levels and effective date.

When working on a self-funded proposal, you should be looking to be at least sixty (60) days before the effective date.

Stop-loss carriers require at least ten (10) working days, once the submission has been received, to release their rates.
