Preferred United Plans® "Alternative Funding" (PUP AF) is a self-funded aggregate stop-loss program designed by SecureOne Benefit Administrators, its actuaries and with Companion Life Insurance Company (AM Best A+ Superior) as the Stop-Loss Carrier.

EMPLOYER
- Must be a Michigan based Employer
- Size of the Employer can be from 2-140 employees.
- May have employees located outside of Michigan as long as they do not exceed 25% of the total enrollment.
- States where employees cannot be covered in are: NY, NJ, MA or CA.
PARTICIPATION REQUIREMENTS
- 2-5 EE’s – 100% Participation - regardless of the reason for waiving (i.e. Spousal or Parental waiver or Medicare).
- 6-10 EE’s – Must have a minimum of 5 enrolling in coverage - regardless of the reason for waiving (i.e. Spousal or Parental waiver or Medicare).
- 11-140 EE’s – Must have a minimum of:

- Employee Health Questionnaire - MICHIGAN
REQUIREMENTS FOR A SOLD GROUP
- Group Employee Applications completed and signed by each Employee, as well as Employees in their waiting period, if their effective date is within 60 days of the group’s effective date. Also, please note that if Employee is married, the spouse must also sign the application, regardless of coverage selected.
- Employer Group Application completed and signed by Employer and agent.
- Copy of Employer’s recent wage & tax report
- Check for first month’s premium made payable to: Preferred United Plans.
- Copy of Current Billing Statement.
- Copy of HRA document (if applicable)
- Copy of PUP “Alternative Funding” quote.
- Group Employee Applications completed by all COBRA Participants.
